Non-interactive version (pdf): How to download, print, & complete:

1. If your computer does not have a pdf reader, download Adobe Reader (free) here.

2. Click on "Download Residency Certification form (pdf)" (right).

3. Print the form.

4. Use ink or typewriter to complete the printed form, and sign it.

5. Send to the Admissions Office.

Interactive version (using Microsoft Word): How to download, complete, & print:

1. After clicking "Download interactive Residency Certification form (doc)" (right), either save to your computer drive or open. 

2. If asked for a password at any time, select "cancel." You can then proceed.

3. Complete the form on your computer.

4. Print the completed form and sign it.

5. You may also print the form as is and use ink or typewriter to complete it.

6. After downloading, close the window.

7. Send to the Admissions Office.
