Non-interactive version (pdf): How to download, print, & complete:

  1. If your computer does not have a pdf reader, download Adobe Reader (free) here.

  2. Click on "Download Application form (pdf)" (right).
  3. Print the Application Form.
  4. Use ink or typewriter to complete the printed form, and sign it.
  5. Send or bring it to the Admissions Office.
  

 

Interactive version (using Microsoft Word): How to download, complete, & print:

  1. After clicking "Download interactive Application form (doc)" (right), either save to your computer drive or open.

  2. If asked for a password at any time, select "cancel."  

  3. Complete the form on your computer.

  4. Print the completed form and sign it.

  5. You may also print the blank form and use ink or typewriter to complete it.

  6. Send or bring your completed form to the Admissions Office.
